Compass Health Systems provides services to individuals with disabilities at every stage of life and recognizes the importance of not only providing services to children with disabilities and unique needs but also young adults as they transition into adulthood and beyond. We realize that each individual comes with unique experiences and needs. CHS seeks to empower individuals with disabilities and their families to live happy, thriving, and purposeful lives on their own terms by empowering them with the tools they need to achieve greater independence and to become integral members of their families and communities. Services available to individuals and families: - Behavior Supports - Community Participation Supports (CPS) - Companion Services - In-Home and Community Supports (IHCS) - Respite Services Behavior Supports Behavior Support Services are a direct and indirect service provided by an experienced, highly skilled BCBA or Licensed Behavior Specialist in the individual’s home or community setting. These services include functional assessment and the development of strategies to support the individual and training of individuals, staff, parents, and caretakers. The purpose of Behavior Support Services is to enable and empower the individual to make the behavioral changes necessary to achieve their goals by observing behaviors, collecting data, developing a behavior support plan, and ultimately transferring skills to the individual and those around them. In-Home and Community Supports In-Home and Community Services (IHCS) is a direct service provided in-home and community settings to aid individuals in acquiring, maintaining, and improving the skills necessary to live independently and participate more meaningfully within their communities. Services may include but are not limited to support and guidance in self-care, health maintenance, decision making, and home management. Respite Services Respite services staff provide relief to caregivers in their absence both within or outside of the individual or individual’s family’s home while keeping the individual engaged in engaging and purposeful activities. Companion Services Direct Support Professionals provide companion services in the home, work, or community settings of the individual. These services include but are not limited to ensuring the individual’s health, safety, and social/emotional welfare in order to perform activities of daily living and help the participant in their communities in a meaningful way. Community Participation Supports The goal of Community Participation Supports (CPS) is to get individuals involved in community activities with individuals with and without disabilities alike.
